Short Term Trip Registration

Thank you for interest in Short Term Mission with GSMRI! Please complete the application below. Before you get started, you may want to gather the following: If you are NOT part of a church team, you will need a Pastoral Letter of Recommendation. This letter can be a document file or scan, and should be uploaded when prompted. (Acceptable formats: .pdf, .doc, .docx, rtf, .txt, .jpg) A scan or file is preferable, but if you are unable to comply, you may also fax the letter after completing this application. A scan of your current Passport. (.pdf or .jpg format, not to exceed 1Mb in size) A scan is preferable, but if you are unable to comply, you may also "snail mail" a color copy of your passport after completing this application.
